Summary of Position

The Youth Services Coordinator works within a team-oriented and fast-paced environment. An integral member of the Unlimited Possibilities for Youth (UP4Youth) program team, this position plays a direct role in supporting and empowering youth ages 16+ with histories of involvement in child welfare as they transition into adulthood. The Youth Services Coordinator provides ongoing case management, resource coordination, and emotional support for the youth, and works with them to develop an individualized service plan (ISP). In addition to individualized support, the coordinator manages our youth support groups, matches youth with adult mentors, promotes events and workshops, and maintains the social media platform(s) related to youth resources.

 

Duties and Responsibilities

  • Enroll youth ages 16+ into the program. Work in collaboration with the UP4Youth Program Specialist to identify eligible youth and enroll them in the program. Complete individualized support plans with youth upon enrollment to assess/establish a baseline. Conduct youth orientations.
  • Provide ongoing case management to former foster youth enrolled in our program. Manage individualized support plans, checking in regularly with youth on their progress toward self-identified goals and connecting youth to/helping them navigate resources relevant to their situation. Preform crisis intervention. Work with youth to develop job-building skills, defined employment goals, and obtain/maintain employment. Support their educational advancement.
  • Match youth with adult mentors from similar backgrounds, culture, and/or experiences. Work in collaboration with the UP4Youth Program Specialist to identify and match mentors with youth, keeping in mind the youth’s unique experiences and employment/education/life goals.
  • Facilitate weekly youth support groups. Promote weekly support groups among youth and ensures topics of discussion that support their unique, collective experiences.
  • Support youth workshops and events, ensuring turnout and active engagement. Work in collaboration with the UP4Youth Program Specialist and the Development team to promote/communicate events to our youth. Support execution of workshops/events.
  • Manage and maintain participant files. Regularly update and document youth progress toward goals using our internal database. Ensure frequent input of case notes and quantitative/qualitative outcomes into the system.
  • Maintain social media account(s) and other resource platforms for the UP4Youth program. In collaboration with the Development team, post on social media all relevant resources/events/communications available to our youth.
  • Perform additional tasks and projects as directed by the Director of Programs.

 

Minimum Qualifications

 KNOWLEDGE AND SKILLS: 

  • Strong knowledge of issues related to foster care, adoption and/or at-risk youth services
  • Able to work with/relate to diverse populations and persons of differing experiences
  • Excellent interpersonal communication, with the ability to communicate across departments and hierarchal structures
  • Able to multi-task the execution and oversight of simultaneous activities and events
  • Sound decision-making capability and ability to exercise independent judgement
  • Maintain and execute confidential information according to HIPAA standards
  • Obtain satisfactory Live Scan Background Check
  • Obtain and maintain CPR/First Aid Certification
  • Able to pass post offer Tuberculosis (TB) and health clearances
  • Must have and maintain a valid California Driver’s License and insurance

 

EDUCATION AND EXPERIENCE:

  • Position requires high school diploma or GED coupled with 1-2 years of post-secondary education and 3-4 years of relevant experience. Bachelor’s degree is preferred.
  • Experience in individualized case management and working with at-risk youth is required.
  • Experience in coordinating support groups, workshops, and events, positively representing an organization/business in public spaces is preferred.
  • Experience with databases such as Apricot or Salesforce is preferred.
  • Experience with social media platforms (Instagram and Facebook) and basic computer skills is required.

Employment Classification

  • FLSA Status: Non-Exempt
  • Salary Range: $45,000 - $50,000 per year
  • Work Schedule: Full-time
